`. .woe <br /> VIII. ENTRY PROCEDURES <br /> 1. The permit space must be purged and continuously vented with <br /> mechanical blowers as required by the Safety Department. <br /> 2. Cleaning shall be done from outside the permit space whenever possible <br /> to preclude the necessity to enter the space. <br /> 3. Spark resistant tools and explosion proof electrical equipment and <br /> lighting must be provided if flammables are used and/or are present at <br /> concentrations greater than or equal to 10% of the LEL. <br /> 4. A secured entry and exit ladder shall be available and used. <br /> 5. Appropriate approved respirators shall be provided and wom when air <br /> contaminants are present, suspected, or could be produced due to <br /> operations conducted within the permit space. <br /> 6. At least one Attendant shall be provided on the outside of the permit- <br /> required confined space to monitor Authorized Entrants. <br /> • The Attendant shall be stationed outside with a clear view of all activity <br /> in the space. <br /> • No other job duties shall be assigned to the Attendant. <br /> • The Attendant shall either be within verbal range of a third party or be <br /> provided with a phone or radio to contact emergency personnel in the <br /> event of an emergency. <br /> 7. Employees shall be provided with and required to use appropriate per- <br /> sonal protective equipment when entering the confined space. <br /> 8. The air in the permit space shall be tested from the outside to determine <br /> whether flammable, toxic, or oxygen deficient atmospheres exist. <br /> ADNIMS/CONSP 13 June 2, 1994 <br />
