During the monthly inspection, the designated <br /> operator shall check that all required testing and maintenance for the UST system have been completed and <br /> document the dates they were done. Ensure that designated operators performing monthly inspections at this facility <br /> are including all of the required information on the reports. <br /> The designated operator failed to document all the alarms from the attached alarm history on the December 2015, <br /> March 2016-May 2016 designated operator monthly inspection report and failed to check that they were responded <br /> to appropriately. The missing alarms include: <br /> -T3:87 Overfill on December 23, 2015. <br /> -T4: Diesel South Overfill on May 4, 2016, May 5, 2016 and April 3, 2016. <br /> - LS:Dispenser 3-4 Dispenser Pan Fuel Alarm on May 31, 2016. <br /> -L18: 91 STP Fuel Alarm on April,9, 2016 and March 13, 2016. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 309 CCR 2636(f)(1) Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The 91 STP sump sensor failed to activate an audible and visual alarm when tested to ensure continuous monitoring <br /> of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the <br /> flow of product at the dispenser when it detects a leak. The sensor was replaced and retested during the inspection. <br /> This is a repeat violation, Class ll. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the: <br /> - 100 Octane on the south side <br /> -91 STP on north side <br /> -Diesel STP on the south side <br /> If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed <br /> of properly. The service technician removed 1/2 gallons of liquid from the STP sumps. Ensure that all sumps and <br /> annular spaces are maintained free of liquid. <br /> This is a repeat violation, Class II. <br /> Page 5 of 6 <br />
