The monitoring plan is not current and/or not approved by the EHD. In the monitoring plan for TA0515700,the monitoring panel was listed as TLS 350 however it was observed as TLS 350 plus.The section labeled"Under Dispenser Containment(UDC)monitoring", labeled"UDC Secondary Containment Monitoring,"is as marked liquid, it should be marked none. No interstitial secondary monitoring was observed at the under dispenser containment on site.The section labeled personnel responsibilities showing Edgar Tracy and Casey Paoletti,should be update according to Mr. Jesus Jurado as they are no longer with the company. Monitoring plan stated daily visual piping inspections are being conducted however Mr.Jesus Jurado stated that the site is doing daily air quality related inspections and not daily visual piping inspections. The monitoring plan for TA0515701,the monitoring panel is listed TLS 350, however, it is observed TLS 350 plus. The section labeled personnel responsibilities showing Edgar Tracy and Casey Paoletti, should be update according to Mr. Jesus Jurado as they are no longer with the company. The monitoring plan must be uploaded to the California Environmental Reporting System(CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. Monitoring plan stated daily visual piping inspections are being conducted however Mr.Jesus Jurado stated that the site is doing daily air quality related inspections and daily visual piping inspections. This was corrected on site. This is a repeat violation, Class II. 107 CCR 2632(d)(1)(C)Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted.The site plan is lacking leak detector locations, piping layout,and the location of the 87 and 91 split tanks are not accurately depicted.The location of the split tanks are reversed A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a repeat violation,Class II. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Accurate monitoring records for the last three years were not found on site. The recent Secondary Containment Testing results located on site for testing conducted on 8/26/16 was not accurate as the date marked on the testing report was marked as 8/26/2016 and 9/22/2016. Accurate dates must be written.The reviewed monitoring plan indicated that the annual tank integrity testing(23 CCR 2643.1)were conducted however results were not found on site.These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records;(3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment,ground water elevations, or other test results;(6)results of inventory readings and reconciliations. Immediately locate the accurate Secondary Containment Testing results and submit a copy to the EHD. This is a Class II violation.
