(Newark-Sierra), Harding Lawson Associates(HLA)submits <br /> a completed application package for a Permit to Close Underground Storage Tank(UST)for a 210,000- <br /> gallon UST containing Number 6 fuel oil and located at the Newark Sierra Paperboard Mills, 800 West <br /> Church Street in Stockton,California(Site). An expedited turn around time of 5-days maximum is <br /> requested and 1.5 times the normal permit fees are submitted with this application package. In <br /> compliance with the application package provided to HLA by the San Joaquin County Public Health <br /> Service Environmental Health Division(PHS/EHD) the following items are submitted to PHS/EHD: <br /> • Detailed Site map including UST location,piping, adjacent streets, site buildings, onsite well <br /> locations. Underground Service Alert(U.S.A.) will be contacted no less than 48 hours prior to any <br /> excavation activities. <br /> • Completed Application for Underground Storage Tank Closure Permit <br /> • Completed, signed, and dated Authorization to Release <br /> • Completed Underground Storage Tank Program Fee Worksheet <br /> • A check made out to PHS/EHD for$351.00 in permit fees for expedited turn-around-time of 5-days <br /> maximum. <br /> • Completed Underground Storage Tank Disposition Tracking Record <br /> • Completed pages 4, 5,and 6 of Application Package <br /> • A site-specific Site Health and Safety Plan <br /> • Copy of City of Stockton Fire Prevention Division Underground Storage Tank Removal Permit;Permit <br /> number 98-96,issued on April 30, 1998 <br /> • A Work Plan for the Complete Removal of the UST <br /> In accordance with Uniform Building Code STD 70-11,because the Site is within the city limits of <br /> Stockton, a Backfill Excavation Certificate is not required. Because the Site is not within the city of Tracy, <br /> a Grading and Excavation Permit is not required by the City of Tracy Building Division. <br /> The closure of this UST will be accomplished by removal of the concrete sides and bottom of the UST <br /> and the wooden roof.
