(Newark-Sierra),Harding Lawson Associates(HLA)submits <br /> to San Joaquin County Public Health Service Environmental Health Division(PHS/EM)an Addendum to <br /> the Application for Permit to Close Underground Storage Tank(UST)submitted by HLA on June 5, 1998 for <br /> the UST located at the Newark Sierra Paperboard Mills, 800 West Church Street in Stockton,California <br /> (Site). This Addendum includes items and information requested by PHS/EHD in a telephone conversation <br /> with HLA on June 10, 1998. In response to the PHS/EHD request the following are submitted to PHS/EHD: <br /> • Completed PHS/EHD contractor's questionnaire for Ogiso Environmental(OGISO), a copy of OGISO's <br /> current certificate of workers compensation insurance,and a copy of OGISO's Hazardous Substance <br /> Removal Certification. <br /> • The corrected EPA Site#is CAD-981375157. <br /> • Completed Backfill Excavation Certificate <br /> • Revised Health and Safety Plan <br /> • The closure report submitted to PHS/EHD will be signed by a California Registered Geologist. The <br /> sampling performed on the stockpiles and the confirmation samples collected from the UST excavation <br /> will be performed under the supervision of a California Registered Geologist. <br /> • In regard to page 4 item 7c Decontamination Method: Steam cleaning with a 3,000 PSI steam cleaner <br /> with rotating jet nozzles and Hotsy Ripper R detergent and this will be done at least three times to <br /> achieve Triple Rinsing. <br /> • In regard to page 5 item I to: Excavation will not be lined;the stockpile will be placed on and covered <br /> by visqueen pending the analytical results on stockpile confirmation samples. If results indicate non- <br /> detectable concentrations or levels acceptable to PHS/EHU the stockpile will be used as backfill <br /> material. The backfill materials to be used do not include asphaltic concrete,asphaltic material,or the <br /> concrete from the sidewalls and bottom of the UST. <br /> • Onsite fill material consisting of insitu clayey silt and clayey sand soils and pieces of concrete which <br /> have been crushed are proposed for use backfill in the UST excavation. Onsite equipment"T-Rex jaws" <br /> will be used to crush all concrete pieces and produces four-inch minus crushed concrete. The material <br /> was derived from demolition of onsite concrete structures. The soils are not contaminated. The <br /> material does not include wood,asphaltic concrete, asphaltic papers,fibers, or other deleterious <br /> substances. <br />
