The following is an itemized list of underground storage tank violations that have not been addressed for ARCO am/pm# 83230 as of December 07, 2017. Open violations from October 24,2017 inspection Violation#101 -No permit to operate. A permit to operate the UST system has not been issued. No person may own or operate an UST unless a permit for its operation has been issued by the local agency to the owner or operator of the UST system. Immediately obtain a permit to operate a UST system from the EHD. An"Operating without a Permit" penalty applies. Violation#121 -Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on 10/20/16 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted within thirty days of receiving the inspection report. This information has not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. Violation#314-Failed to have secondary containment or secondary containment not tight. The inner sump lids for the 87-octane, 91-octane, and diesel submersible turbine pump(STP)sumps do not have clamps and sump gaskets are missing from the three fill sumps.Also, holes were drilled into the collars of the 87-octaine, 91-octane, and diesel fill sumps and lids to install the sump lid clamps. Secondary containment shall be impervious to the liquid and vapor of the substance contained and constructed to prevent structural weakening as a result of contact with any hazardous substances released from the primary containment. Immediately contact a properly licensed,trained, and certified contractor to make repairs to the UST system under permit and inspection of the EHD. A Return to Compliance Certification was received at the Environmental Health Department(EHD)on 11!3115 but violations 317(Secondary containment not tight)and 320 (Secondary containment not constructed to prevent water intrusion)have not been addressed resulting in a non-compliant status for this facility. The inner sump lids for the 87-octane, 91-octane, and diesel submersible turbine pump(STP)sumps do not have latches and sump gaskets are missing from the three fill sumps. All four of the 87-octane spill and vapor container reducer boots were missing the upper band clamps and one of the reducer boots was missing the lower band clamp,too. Secondary containment shall be impervious to the liquid and vapor of the substance contained and constructed to prevent structural weakening as a result of contact with any hazardous substances released from the primary containment. Immediately contact a properly licensed, trained, and certified contractor to make repairs to the UST system under permit and inspection of the Environmental Health Department(EHD).
