CASE SUMMARY <br /> On September 23, 2002, secondary containment testing was performed on the 10,000- <br /> gallon diesel underground storage tank (UST) by Shirley Environmental Testing at the <br /> above-referenced facility, and a report was submitted to this office on October 4, 2002, <br /> (Attachment 1). The secondary piping runs did not pass. <br /> On February 19, 2003, a permit application, SR0032811, was submitted by Shirley <br /> Environmental Testing to repair the failed secondary containment components. The <br /> application was approved by Ms. Dina Abate, Registered Environmental Health <br /> Specialist (REHS), San Joaquin County Environmental Health Department (EHD), on <br /> March 3, 2003. This permit application cannot be found in the EHD's files. <br /> On November 13, 2003, Mr. John Jackson, REHS, EHD, performed a routine UST <br /> inspection (Attachment 2) and witnessed the annual monitoring system certification <br /> (Attachment 3). During the inspection, Mr. Jackson found the EHD did not have current <br /> UST — Facility and UST — Tank forms for this facility on file, monitoring and response <br /> plans had not been completed, and maintenance records were not available for <br /> inspection. The inspection report did not properly document that the secondary <br /> containment failures had not been addressed and that the approved permit application, <br /> SR0032811, had expired. <br /> On December 2, 2003, testing was performed by L.A. Perks Plumbing and Heating Inc., <br /> (L.A. Perks) to find the source of the secondary containment failures. No repairs had <br /> been made at this point, and the secondary piping runs still did not pass inspection. It <br /> was noted in the report that the supply line had a leak underground and the return line <br /> had a leak inside the building (Attachment 4). This testing was performed for diagnostic <br />
