purposes only and did not meet triennial secondary containment testing requirements <br /> because the EHD was not notified prior to testing. <br /> On December 16, 2003, Mr. Jackson called and spoke with Ms. Nora Galindo, <br /> Environmental and Health Manager for Sprint. Ms. Galindo stated that they fired Shirley <br /> Environmental Testing and hired L.A. Perks to make the necessary repairs. Ms. Galindo <br /> also stated that L.A. Perks will submit a permit application for repairs. <br /> On January 26, 2004, Ms. Galindo submitted a package of completed forms that were <br /> requested during the November 13, 2003, routine UST inspection. The cover letter <br /> stated a current UST — Facility (former Form A) was included, but was not part of the <br /> submittal (Attachment 5). <br /> On January 26, 2004, a permit application, SR0036721, was submitted by L.A. Perks to <br /> repair the secondary containment failures. This permit was approved by Mr. Jackson on <br /> January 30, 2004 (Attachment 6). <br /> On March 3, 2004, Ms. Michelle Le, REHS, EHD, received a call from Randy of L.A. <br /> Perks stating that the failures were too extensive to be repaired and that the piping <br /> needed to be replaced. He also commented that he would not put his name on such a <br /> patch job and had covered up the pipe and was leaving the job site. According to <br /> Randy, Sprint would rebid the job as a piping removal/install. <br /> On March 4, 2004, Ms. Le called and asked Ms. Galindo about the secondary <br /> containment repairs. Ms. Galindo said she would choose a contractor by the next day <br /> and submit a permit application to the EHD. <br /> On April 6, 2004, a permit application, SR0037495, was submitted by Stockton Service <br /> Station Equipment Company, Inc. to repair the failed secondary containment <br /> components and was approved by Mr. Jackson on April 16, 2004 (Attachment 7). <br /> On May 11, 2004, photos were taken of portions of the piping as they were uncovered <br /> for repairs (Attachment 8). <br /> On May 26, 2004, Mr. Jackson witnessed the pressure soap test of the two secondary <br /> lines and verified the appropriate slope of the piping, which was not required for this type <br /> of repair. One of the lines did not hold pressure; the other passed. <br /> On May 28, 2004, Mr. Jackson witnessed the retest of the secondary lines. Mr. Randy <br /> Chadwick and Mr. Mike Jones of Stockton Service Station Equipment Company stated <br /> that a leak was found on the piping near the piping sump. The fitting was replaced and <br /> both lines were retested and passed. Photos were taken of the repairs (Attachment 9). <br /> On June 9, 2004, Mr. Jackson received a call from Mr. Eric Vannamaan of RIC <br /> Construction. Mr. Vannamaan stated that his company hired Stockton Service Station <br /> Equipment Company to repair the secondary failures and requested Mr. Jackson provide <br /> a statement that the failures have been repaired. Mr. Jackson stated he would send an <br /> inspection report indicating what he witnessed. On June 11, 2004, Mr. Jackson faxed <br /> Mr. Vannamaan the inspection report(Attachment 10). <br /> 2 <br />
