Secondary containment testing was last performed on May 7, 2007 and was completed on September <br /> 22, 2010, 4 months past due. Secondary containment testing is required once every 36 months. Take <br /> all necessary precautions to ensure testing is performed in a timely manner. <br /> 2. Secondary containment testing was performed on September 22, 2010 and a test report has not been <br /> submitted to the EHD. A copy of the test report must be submitted within 30 days of the test. <br /> Immediately submit a copy of the test report to the EHD. Note: Copy provided at time of inspection. <br /> 3. UST facility form on file with the EHD is not current. Any change of information on these forms must <br /> be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the <br /> facility form to the EHD. AWk, <br /> 4. Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility by June 28, 2011. <br /> 5 & 6. Current approved copies of the monitoring and response plans were not found on site. The <br /> monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br /> of the monitoring and response plans for approval by the EHD by June 28, 2011. An approved copy will <br /> be returned for your records. <br /> 23c. The designated operator failed to document all the alarms for the previous month and check that <br /> they were responded to appropriately on the October 2010 designated operator monthly inspection <br /> report. The missing alarms include: L1 STP Sump fuel alarm September 22, 2010 and September 25, <br /> 2010. During the monthly inspection, the designated operator shall review the alarm history for the <br /> previous month, check that each alarm was documented and responded to appropriately, and attach a <br /> copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. <br /> 23c. The designated operator failed to visually inspect the STP Sump for leaks, water, or debris during <br /> the October, 2010 monthly inspection. During the monthly inspection, the designated operator shall <br /> visually inspect all spill containers and UDC sumps, and any tank top sump that had an alarm for which <br /> ALL EHD STAFF TIME ASSOCIATED VITTH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> EHD Inspector: Reoelved By: Tsie' <br /> EHD 2&02 8 Rev OeH 10 CONTINUATION FORM <br />
