The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. If contamination is found in the UST pit, the new UST system cannot be placed into the <br /> contaminated pit until the EHD Unit 4 Local Oversight Program (LOP) receives and approves a <br /> work plan for remediating the contamination. <br /> 3. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site, at all times while work is being performed, who <br /> possesses all of the following: <br /> One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), and <br /> • A valid, current UST Installation/Retrofitting certificate issued by the International Code <br /> Council (ICC), and <br /> A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being installed. <br /> Additionally, <br /> Each person installing or testing any component must hold a certificate of training from the <br /> manufacturer of that component. <br /> 4. Qualified service technicians must possessor work under the direct and personal supervision of <br /> an individual physically present at the work site, at all times while work is being performed, who <br /> possesses all of the following: <br /> • One of several permissible valid, current contractor licenses issued by the Contractors <br /> State License Board (may be issued to the installation company), and <br /> • A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> • A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being tested, repaired, or serviced. <br /> Pagel of 3 <br />
