Overfill prevention for the UST shall satisfy CCR Section 2635(b)(2)(A) pursuant to the permit application. 16.All UST metal components in direct contact with the ground such as flexible connectors, swing joints, fittings, clamps, bands, and pumps shall have corrosion protection. 17.Any changes in the approved scope of work shall be submitted to the EHD for approval prior to commencing work. All new submittals supersede the previous ones. 18.A"Certification of Installation/Modification" form for each tank shall be submitted by the owner or the owners' agent [pursuant to CCR Section 2635(d)] before this install will be considered complete by the EHD. 19.An accurate and drawn to scale as-built plan must be submitted to the EHD prior to scheduling a final inspection. 20. No gasoline, diesel, or other hazardous material or waste can be released (intentionally or unintentionally) to the sewer/septic, storm water system, or any other waters of the state. If a release occurs it will be considered illegal hazardous waste disposal and will be subject to enforcement. 21. Submit the Best Management Practices (BMP) to the EHD for managing fuel spills and wash- down water at this site prior to the final inspection. 22. Submit correct and complete Monitoring and Response Plans for approval prior to scheduling a final inspection. 23. Employee training by the UST Designated Operator on the new UST system must be completed and documentation submitted to the EHD prior to scheduling a final inspection. 24. Submit a Certification of Financial Responsibility for Underground Storage Tanks containing Petroleum Form with the appropriate mechanism prior to scheduling a final inspection. 25. Passing Enhanced Leak Detection (ELD) results for the UST must be received by the EHD prior to depositing product into the UST. Page 2 of 3
