Personal Protective Equipment Failure: If any site worker experiences a failure or <br />alteration of protective equipment that affects the protection factor, that person shall <br />immediately leave the Exclusion Zone. Reentry shall not be permitted until the <br />equipment has been repaired or replaced. <br />Other Equipment Failure: If any other equipment on site fails to operate properly, <br />the SHSC shall be notified and determine the effect of this failure on continuing <br />operations on site. If the failure affects the safety of personnel or prevents <br />completion of the Work Plan tasks, all personnel shall leave the Exclusion Zone <br />until the situation is evaluated and appropriate actions taken. <br />8.4 Emergency Response Follow -Up <br />The SHSC shall submit an incident report to the Project Manager. The report shall <br />include the following: <br />• Chronological history of the incident <br />• Facts about the incident and when they became available <br />• Titles and names of personnel <br />• Actions: decisions made and by whom, order given, to whom, by whom and <br />when. Action Taken, who did what, when where, and how. <br />• Possible exposure of site personnel <br />• History of all injuries or illnesses during or as a result of the emergency. <br />
