Periodic Training: <br /> Employees shall additionally receive specific on-site follow-up training <br /> as needed by the Certified Industrial Hygienist The CIH shall provide <br /> follow-up training at least weekly, on problems observed during the <br /> previous week such as improper use of respirators or protective <br /> clothing, violation of decontamination procedures, previous week' s air <br /> monitoring results or other discrepancies. The CIH shall additionally <br /> provide special training when unanticipated problems or changes in the <br /> cleanup operations, SSHP and Activity Hazard Analyses occur, and to new <br /> employees as needed. <br /> MEDICAL SURVEILLANCE : <br /> The Safety and Health Manager, in conjunction with the Occupational <br /> Physician, shall detail. in the employer's Safety and Health Program and <br /> the SSHP, the medical surveillance program that includes scheduling of <br /> examinations, certification of fitness for duty, compliance with OSHA <br /> requirements, and information provided to the physician. Examinations <br /> shall be performed by or under the supervision of a licensed physician, <br /> preferably one knowledgeable in occupational medicine, and shall be <br /> provided without cost to the employee, without loss of pay and at a <br /> reasonable time and place. Medical surveillance protocols and <br /> examination and test results shall be reviewed by the Occupational <br /> Physician. The medical surveillance program shall contain the <br /> requirements specified below. Personnel working in contaminated areas of <br /> the site shall have been examined as prescribed in CFR 29 Part 1910 <br /> Section .120, and CFR 29 Part 1926 Section . 65, and determined medically <br /> fit to perform their duties. <br /> Frequency of Examinations: <br /> Employees shall have been provided with medical exams within the past 12 <br /> months and shall receive exams annually thereafter (if contract duration <br /> exceeds 1 year) ; on termination of employment; reassignment in <br /> accordance with CFR 29 Part 1910 Section .120 (e) (3) (i) (C) , and CFR 29 <br /> Part 1926 Section .65 (e) (3) (1) (C) ; if the employee develops signs or <br /> symptoms of illness related to workplace exposures; if the physician <br /> determines examinations need to be conducted more often than once a <br /> Year; and when an employee develops a lost time injury or illness during <br /> the period of this contract. The supervisor shall be provided with a <br /> written statement signed by the physician prior to allowing the employee <br /> to return to the work site after injury or illness resulting in a lost <br /> workday, as defined in CFR 29 Part 1904 Section .12 (f) . <br /> Content of Examinations: <br /> The following elements shall be included in the medical surveillance <br /> program.
