SITE SAFETY 6 HEALTH PLAN REQUIREMENTS FOR PERSONAL PROTECTIVE <br /> ' EQUIPMENT (SSHP PPE Requirements) : <br /> GENERAL: <br /> 1 . The following requirements apply to all operations and <br /> personnel who might be exposed during any hazardous <br /> tasks: <br /> 2 . The contractor shall provide respirators and other PPE <br /> necessary to protect the health of all employees . <br /> 3. The contractor shall provide PPE for three government <br /> personnel on a daily basis. <br /> 4 . The SHM shall determine the degree of exposure through <br /> industrial hygiene environmental surveillance and select <br /> PPE based on the results of the monitoring, site <br /> observations and professional judgment. <br /> 5. The contractor shall establish and maintain a <br /> respiratory protection program per 29 CFR 1910.134. <br /> 6. The contractor shall use only NIOSH/MSHA approved <br /> respirators. <br /> 7. Each person wearing respiratory protection shall have a <br /> physician's certification of fitness prior to being <br /> issued a respirator. <br /> 8. All persons wearing respirators shall have been fit <br /> tested prior to commencing hazardous operations. <br /> 9. Each person wearing a respirator shall have been briefed <br /> in proper procedures of donning and removing, <br /> decontamination, maintenance and storage of the <br /> respirator or other personal protective devices. <br /> 10. All persons wearing a respirator shall have annual <br /> training in the proper use and care of the respirator. <br /> 11 . Each employee shall have their own individual <br /> respirator. No person shall wear the respirator of <br /> another employee. <br /> Only respirators approved by NIOSH shall be used. Onsite personnel shall <br /> be provided with appropriate personal protective equipment. Protective <br /> equipment and clothing shall be kept clean and well maintained. The PPE <br /> section of the SSHP shall include site-specific procedures to determine <br /> PPE program effectiveness and for onsite fit-testing of respirators, <br /> cleaning, maintenance, inspection, and storage of PPE.
