The following is an itemized list of underground storage tank violations that have not <br /> been addressed for VAN DE POL ENTERPRISES INC as of October 05, 2017. <br /> Open violations from January 25,2016 inspection <br /> Violation#209-Designated operator did not inspect all required items andlor the inspection reports not <br /> completed. <br /> -The designated operator failed to attach alarm history printouts to the January 2015 and September 2015 monthly <br /> inspection reports. <br /> -The designated operator failed to print the alarm history for the T1 and T2 tank alarms. <br /> -The designated operator failed to check that all alarms were responded to appropriately. The missing alarms <br /> include: <br /> L5(diesel annular)Sensor out 511115 <br /> L5 (diesel annular)Sensor out 514115 <br /> L6 (87 STP sump) Fuel alarm 9120/15 <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> Violation#210-Designated operator monthly inspection reports not maintained on site for at least 12 <br /> months. <br /> The July 2015 through present designated operator monthly inspection reports were not found on site. Designated <br /> operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure <br /> that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. <br /> Submit copies to the EHD. <br /> Violation#314-Primary containment not tight(pre-Jul 2003). <br /> Drippage was observed on the west piping under dispenser 516, indicating a leak in the primary piping. All primary <br /> containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified <br /> contractor repair or replace the failed component under permit and inspection of the EHD. <br /> Violation#319-Water in secondary containment not removed,analyzed,and properly disposed of(pre-Jul <br /> 2003). <br /> Liquid was observed in all three UDC sumps. If water could enter into the secondary containment by precipitation or <br /> infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste <br /> determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the UDC sumps <br /> are maintained free of liquid. <br /> Violation#325-Spill container failed to contain a minimum capacity of five gallons. <br /> The 91 spill container failed when tested. All spill containers shall have a minimum capacity of five gallons and be <br /> capable of containing a spill or overfill until it is detected or cleaned up. The spill container O-ring was replaced and <br /> the container was retested during the inspection. <br /> Page 3 of 3 <br />
