The following is an itemized list of underground storage tank violations that have not <br /> been addressed for VAN DE PUL ENTERPRISES INC as of October 05, 2017. <br /> Open violations from January 31,2017 inspection <br /> Violation#313-Failure to construct,operate,and maintain primary containment as product-tight. <br /> Approximately 1-2 gallons of product was observed in UDC 314, and <0.5 gallons of product was observed in UDC <br /> 516, and 1-2 cups of product was observed in UDC 112, indicating multiple leaks in the primary piping. All primary <br /> containment for the UST system must be tight.Any release from the primary containment that does not escape the <br /> secondary containment and was cleaned up within eight hours of being detected shall be recorded, maintain all. <br /> required recordable release documentation. Immediately have a properly licensed, trained, and certified contractor <br /> repair or replace the failed component under permit and inspection (as applicable) of the EHD. Submit proof of <br /> correction to the EHD. <br /> Open violations from January 25,2016 inspection <br /> Violation#104-UST Operating Permit Application for Facility and Tank information not submitted or <br /> current, <br /> UST Tank Information forms for all three tanks are not current in CERS. <br /> 87 tank-The tank form incorrectly stated that the tank has double walled vent lines. The vent lines are single <br /> walled. <br /> 91 tank-The tank form incorrectly stated that the tank has double walled vent lines. The vent lines are single <br /> walied. <br /> Diesel tank-The tank form incorrectly stated that the tank has double walled vent and vapor recovery piping. The <br /> vent lines are single walled. Diesel does not have vapor recovery lines. <br /> Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, <br /> update the required information, and submit for review by the EHD. <br /> Violation#201 -Failed to maintain alarm logs and/or records of fallow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. No records were available for <br /> responses to the following alarms: <br /> L5(diesel annular)Sensor out 511115 <br /> L5(diesel annular)Sensor out 514115 <br /> L6(87 STP sump) Fuel alarm 9120115 <br /> These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of <br /> all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br /> observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br /> monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and <br /> reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br /> site and submit copies to the EHD. <br /> Violation#205-Monitoring and response plans not current or approved by the EHD. <br /> The monitoring plan is not current and/or not approved by the EHD. <br /> 87 tank-The UDC monitoring section states it stops the flow of product at the dispenser;the sensor shuts off the <br /> turbine. <br /> 91 tank-The monitoring plan lists two leak detectors and does not specify the specific leak detector model for this <br /> tank. Immediately log <br /> into CERS, make the necessary changes, and submit for review by the EHD. <br /> Violation#206-Operating permit not on site. <br /> A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments <br /> and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. <br /> Page 2 of 3 <br />
