MAJESTIC TECH <br /> Remove tape <br /> Remove outer gloves <br /> Remove suit from inside out <br /> Remove respirator facepiece <br /> - Remove inner gloves <br /> Wash hands , face and any exposed skin <br /> Decontamination supplies to be provided daily at the work site by <br /> Majestic Technologies, Inc. include visqueen plastic, duct tape, <br /> traffic cones, barrier tape, a wading pool or plastic tub (large <br /> enough to stand in with both feet ) , bucket. a long-handled scrub <br /> brush. waste collection containers (open-top 55-gallon drum with drum <br /> liner, or several lawn-size garbage bags) . and Trisodium Phosphate <br /> (TSP) or equivalent soap/decontamination solution. The <br /> decontamination solution shall be made by mixing &;proximately 1/8th <br /> cup of TSP with one to two gallons of water (in a bucket ) , and shall <br /> be made daily for use. The SHO shall ensure that the decontamination <br /> station is set up completely prior to the beginning of work each day. <br /> All field personnel exiting an exclusion zone must pass through a <br /> decontamination procedure before entering the support zone. All waste <br /> generated from decontamination shall be properly packaged and managed <br /> Es a regulated hazardous waste. <br /> In addition to personnel , all equipment and hand tools removed from <br /> the exclusion zone shall be decontaminated by washing or wiping down <br /> with lint-free rags moistened wih decontamination solution. For <br /> backhoes and other contraction equipment , the tires and bucket/blade <br /> shall be cleaned with a wire brust , and all soils collected and <br /> placed with excavated soils . <br /> 12. 3 TAILGATE SAFETY MEETINGS <br /> The SHO shall conduct a Tailgate Safety Meeting with all field <br /> Personnel , including subcontractor personnel, at the beginning of <br /> every shift. This safety meeting shall be documented using a Tailgate <br /> Safety Meeting form ( see attachment 5 ) . tailgate Safety Meetings <br /> shall be conducted and documented in accordance with Mayestic <br /> Technologies. Inc. ' s Health and Safety Manual (see section 1035 ) . The <br /> Tailgate Safety Meeting, and its form, shall be updated whenever daily <br /> site conditions, hazards, or scope of work changes. A copy of each <br /> day 's Tailgate Safety Meeting form shall be maintained at the <br /> immediate work site and shall be available for field personnel and <br /> Site visitors to review. All site visitors are to be briefed on the <br /> operations and daily Tailgate Safety Meeting information prior to <br /> entering a designated exclusion zone. Copies of all Tailgate Safety <br /> Meeting forms shall be maintained on-site during the project , and <br /> Shrill hp nnn,,AAen +.. +tie rnv , -...- _....•....-- <br />
