MAJESTIC TECH <br /> 12 . 4 SAFETY INSPECTIONS <br /> The SHO shall perform a daily safety inspection of the work silte. <br /> The daily' safety inpsections shall be documented using the Safety <br /> Inspection form ( see attchement 6 ) . The daily safety inspections <br /> shall be performed and documented in accordance wih Majestic <br /> Technologies. Inc. ' s Health and Safety Manual (see section 1033 ) . <br /> The SHO shall ensure that all immediate hazards are corrected <br /> before work proceeds and that all other hazards and potential <br /> safety situations are corrected in a timely manner in relation to <br /> this project ( i. e. , one to three days) . <br /> In addition , the SHO shall perform a specific safety inspection of <br /> each piece of equipment to be used that day (i. e. , backhoe, loader, <br /> etc . ) . The equipment inspection shall be documented using a Safety <br /> Inspection Checklist for Construction Equipment (CESPD Form 150-P,: <br /> see attachments 7 ) . Any defensive items or conditions shall be <br /> corrected before the commencement of work. <br /> Copies of all safety inspection forms (i . e. . work site and <br /> construction equipment ) shall be available for field personnel <br /> review, shall be provided to the COR upon request. <br /> 12. 5 ACCIDENT INVESTIGATIONS <br /> In the event of an injury. Illness, or near miss, the incident <br /> shall be immediately reported to the Field Supervisor or SHO. If <br /> required, emergency medical care or first ais shall be rendered . <br /> The Field Supervisor shall then initiate s Supervisor ' s Report of <br /> Injury, and other documents required for injury/illness <br /> reporting and worker ' s compensation claims , <br /> As soon as possible afeter the occuranCe accident, the SHO shall <br /> in=tiate an Accident Investigation in compliance with Majestic <br /> - echnoloZies , Inc. 08 Health and Safety Manua- <br /> the accident investigation shall be documented on an Accident <br /> Investigation form (see attachment 8 ) . The purpose of the accident <br /> investigation is to indify the causes of the incident in order to <br /> take corrective action to prevent future the Incident <br /> Occ <br /> In addition, the SHO shall <br /> reported injury, illness, immediately notify the COR of the <br /> states Army Corps or near miss , and shall of Engineers Accident complete a United <br /> The Investigation Report (see <br /> attachment g) , form shall be provided to the COR as <br /> Possible following the reported incident. <br /> soon as <br /> Finally, the SHO shall maintain an <br /> Occupational Injuries and IllnessOSHA Log and Summary of <br /> es on-site during <br /> project (OSHA Injury <br /> Form) . The lthe entire <br /> og shall <br /> recordable injury or be updated if and when a <br /> OZ <br /> occurs. The log Shall be available <br /> for inspection by the Orange County Project Manager at anytime, <br /> 2. 6 SANITATION <br />
