.. <br />HEALTH AND SAFETY PLAN <br />RE: Tank removal - May & Susan Burton, 4515 Homer St., Stockton, CA 95215 <br />Tank and Product <br />One (1) 550 gasoline tank <br />The project manager and health and safety coordinator is Lou Tvedt of Paydirt Excavating <br />Co. Inc. 269 Oregon St. , Ripon, CA 95366, Phone # (209) 599-2347 Cell number: 209- <br />481-4072. The site safety officer will be Lou Tvedt of Paydirt Excavating Co. Inc. The <br />alternative site of safety officer will be George Gilmore of Paydirt Excavating Co. Inc. <br />Paydirt Excavating Co. Inc., will have on site two (2) fire extinguishers and one (1) first <br />aid kit within 50 feet of tank location (see map). No smoking will be permitted within 50 <br />feet of the tank or areas where flammable vapors are present. Two no smoking signs will <br />be posted at job site. Paydirt Excavating co. Inc. shall notify USA Dig 48 hours before <br />excavation shall begin. Tank will be emptied of product before start of job. We will seal <br />an openings and one (1) plug will have an 1/8 inch opening before tank will be transported <br />along with piping. At no time will Paydirt Excavating co. Inc. cut or weld on site. Tank <br />will be secured on truck after excavation for transportation. The gasoline tank and <br />plumbing will be hauled by Ecology Control Industrial, 255 Parr Blvd. Richmond CA <br />Phone # 510-235-1393. Paydirt Excavating Co. Inc. will approach all tank excavations <br />with possible cave-ins. The following rules will be followed to prevent any mishaps: <br />1. Soil will be inspected for stability before and after removal of tanks. <br />2. Soil to be at least two (2) feet from edge of an excavation. <br />3. If excavation is five (5) feet or more, the hole shall be tapered, braced, or other <br />equipment methods shall be used to prevent possible cave-ins, precautions will be at all <br />times. <br />Soil samples will be taken by Geological Technics Inc., 2741 River Rd. Modesto, CA <br />95351 (209-538-6424). Soil samples will be excavated. Using back hoe, samples will be <br />taken two (2) feet to four (4) feet. By hand using brass tubes, all AB -one procedures will <br />be according to code. Paydirt Excavating co. Inc. will use proper equipment for each tank <br />removal. No undersized equipment will be used. All heavy equipment will be supported <br />properly. Tanks are to be secured at the assigned location after extraction. <br />Paydirt Excavating Co. Inc. will provide safety hats, goggles, and clothing to employees at <br />the job site as per OSHA Regulation 29 CFR 1910, 120. Caution will be noted that a heat <br />stress situation may develop when impermeable protective clothing is worn. Paydirt <br />Excavating Co. Inc. will hold shop meetings before start of tank removals, to discuss <br />
