PAGE 4 <br /> 2. 2 . 2 . 5 <br /> Should medical care be authorized, immediately notify the <br /> office of any injury related accident, no matter how small . <br /> The supervisor should complete the "Supervisor's Report of <br /> Injury" in detail and send to the office within twenty-four <br /> ( 24 ) hours of the incident. <br /> 2 . 3 Equipment/Vehicle Accidents: <br /> 2 . 3 . 1 Office Procedures : <br /> 2 . 3 . 1 . 1 <br /> The insurance carrier's report of accident shall be complete <br /> from information supplied by the superintendent, foreman, or <br /> employee. Be as complete as possible - but do not hold up the <br /> report. <br /> 2 . 3 . 1 . 2 <br /> Notify the current liability carrier and our agent by phone. <br /> Report as much information as you have available. Record the <br /> name , their position, and phone number of the person reported <br /> to, the time reported and so forth. <br /> Equipment/Vehicle/Safety/Accident Reporting - Supervisor's Duties. <br /> 2. 3 . 2 . 1 <br /> Each supervisor shall make all employees aware of procedures , <br /> operating instructions and so forth. At the start of each <br /> project and to inform each new employee at the start of his <br /> job. <br /> 2 . 3 . 2 . 2 <br /> Each employee is to be informed of the following: <br /> A. Only licensed and/or authorized employees shall be permitted <br /> to operate company owned or controlled motor vehicles and/or <br /> equipment. <br /> B. All accidents involving a company owned or controlled motor <br /> vehicle or item of equipment should be reported immediately, <br /> regardless of the nature of the accident, or how trivial it <br /> might appear to the driver or operator. <br /> C. Drivers and/or operators will be held responsible for the <br /> prompt, accurate and complete reporting of any accident in <br /> which they become involved. <br />
