DO failed to document correct test dates. In DO Jan 2013, October - November 2012 reports, the <br />DO noted the secondary containment (2CT) testing as 09115/09. Thus 2CT is past due since the tri- <br />ennial testing should have been due by 09/2012 and DO failed to notify operator of past due testing. A <br />copy of the recent 2CT test result were found and test was performed on 10/23112. DO should have <br />documented the recent test dates on the DO monthly reports starting in October 2012 to present. During <br />the monthly inspection, the designated operator shall check that all required testing and maintenance for <br />the UST system have been completed and document the dates they were done. Ensure that designated <br />operators performing monthly inspections at this facility are including all of the required information on <br />the reports. Submit a statement how DO would ensure correct dates are documented and how past DO <br />monthly reports would be corrected. <br />108. Secondary containment testing was last performed on 09/15/09 and was completed on 10/23/12, <br />one month past due. Secondary containment testing is required once every 36 months. Take all <br />necessary precautions to ensure testing is performed in a timely manner. Submit a statement how to <br />ensure next 2CT is done in a timely manner. Next due by 10/2015. <br />313. All spill containers, 87 Master, 87 Siphon, 91 and diesel, all had approximately 1/4 to 1/2 full of <br />liquid in the buckets. All spill containers shall have a minimum capacity of five gallons and be capable of <br />containing a spill or overfill until it is detected or cleaned up. Immediately remove any liquid in the spill <br />bucket into the appropriate hazardous waste container. The DO is responsible for inspecting spill <br />buckets, documenting and notifying operator if there is any liquid that needs to be removed. Store <br />employees should check the spill bucket prior to any fuel delivery. No fuel deposition should be allowed <br />if there is liquid in the spill bucket as the spill bucket is not prepared to contain a minimum of 5 gallons of <br />spill fuel during an accident. Submit a statement how to ensure spill buckets will be maintained empty at <br />times. <br />Submit to SJC EHD within 30 days (02116/13): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for each of the above violations <br />Note: - Provided: HW labeling instruction, CERS flyer, CERS Lead User Autho form, RTC Cert form <br />- DO documented a December 18, 2012 monthly report with an alarm history printout attachment of the <br />same date.
