Approximately 112 to 1 gallon of liquid was found in 87 Master Fill sump, Diesel Fill sump and 3 <br />cups of liquid in Diesel STP sump seen during annual compliance testing today. In addition, from the <br />Walton Engineering work orders, it appears that the L16 diesel fill sump liquid detection sensor is in <br />alarm often - <br />1. WO # 49352, on 05102112, approximately 3 gallons of liquid was removed <br />2_ WO # 48725, on 04108112, liquid was removed <br />3. WO # 48094, on 03120112, diesel fill sump alarm <br />4. WO # 39975, on 06/08/11, approximately 4 gallons liquid removed form 87 fill & diesel fill sump <br />If water could enter into the secondary containment by precipitation or infiltration, it must be removed <br />and disposed of properly. Immediately remove this liquid, make a hazardous waste determination per <br />Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the Fill sumps and all <br />other sumps are maintained free of liquid. <br />- Corrected on site. Technician removed liquid and placed it into facility's hazardous waste drum. <br />Submit a statement explaining how or if any repairs will be made to prevent any future liquid intrusion <br />into any sumps, especially for the diesel fill sump_ <br />204. Designated Operator (DO) failed to document date of alarm. On a DO May 2012 report, he <br />documented a 05/02/12 for L16 Diesel Fill Sump fuel alarm. The L16 alarm occurred on 04/25/12, <br />03/31/12 and 03/16/12, but no alarm on 05/02/12. There was a Walton Engineering work order for <br />05/02/12 in response to the L16 alarm. DO must document alarm date of Occurrence but reference when <br />action response was taken. During the monthly inspection, the designated operator shall review the <br />alarm history for the previous month, check that each alarm was documented and responded to <br />appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. Submit a statement ensure DO <br />correctly documents date of each alarm occurrence. <br />ALL EHD STAFF TIME ASSOCIATED 141TH FAILING TO COMPLY BY THE ABOVE NOTED DATES MILL BE BILLED AT THE CURRENT HOURLY RATE_ <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />-ate �a i uvJr �, ivry rttruN r <br />
