Lindsay St., Stockton Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS 1,CLASS II,or MINOR-Notice to Comply) <br /> This is a routine underground storage tank (UST) inspection and of the monitoring certification. The <br /> original checklist was provided to facility on June 30, 2010, and the final official inspection report was <br /> hand delivered to facility on July 7, 2010. The following Items #4,5,21 are repeat violations, and #13,17, <br /> 23a are violations observed during the inspection on June 30, 2010: <br /> 4. (Repeat) Current Financial Responsibility (FR) and Chief Financial Officer (CFO) letter were not <br /> provided to this office (EHD). <br /> - Evidence of current FR, and a CFO letter if the State Clean Up Fund is to be used as a part of the FR <br /> for the facility, shall be provided to the local agency. <br /> * Submit evidence of FR to EHD by August 8, 2010. This violation has been corrected on site. <br /> 5. (Repeat) Monitoring Plan (MP) is not current. <br /> - Facility shall retain a current MP approved by EHD on site for inspection. <br /> * Submit a copy of the current MP to EHD for approval by August 8, 2010. This violation has been <br /> corrected and EHD approved the updated MP on site. <br /> 13. No audible alarm when UST#1 STP liquid sensor was tested. <br /> - UST system piping and UDC shall be continuously monitored such that if a leak is detected the flow <br /> of product is shut off(positive shut down), or activates a visual and audible alarm. <br /> * Immediately repair UST system to provide positive shut down, or activates a visual and audible <br /> alarm. Technician reprogrammed the Monitoring Panel and reactivate audible portion of the alarm for all <br /> leak sensors at the facility. This violation has been corrected. <br /> 17. 87 Grade UST line leak detector failed a 3 g.p.h. leak test. <br /> - Line leak detectors shall be able to detect a 3 g.p.h. leak at 10 p.s.i. <br /> * Immediately repair or replace the 87 grade line leak detector under permit by a certified contractor. <br /> Submit the permit application to EHD within 24 hours of repair. The new line leak detector was replaced <br /> on site during the inspector and passed the 3 g.p.h. leak test. Submit the permit application to EHD <br /> within 24 hours. <br /> 21. (Repeat) Observed liquid in secondary containment sumps of UST#6, #7, and #8. <br /> - Water in secondary containment shall be removed, analyzed, and properly managed. <br /> * Immediately remove the liquid in the above secondary containment, make a hazardous waste <br /> determination of he liquid, and properly managed it accordingly. Violation corrected on site. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title. <br /> EHD 23-02-00 L06/040 CONTINUATION FORM <br />
