Failed to notify EHD change of Designated UST Operator (DO). <br /> Facility shall notify local agency any change of DO within 30 days of change. <br /> Submit a copy of the owner statement of DO to EHD by August 8, 2010, and continue this practice in <br /> the future. A copy of the owner statement of the current DO has been submitted to this inspector on July <br /> 7, 2010 on site. This violation has been corrected. <br /> NOTES <br /> All above violations have been corrected by July 7, 2010, with the exception of submitting the Permit <br /> Application to replace the line leak detector.
