Katzakian explained his father used it to <br /> put gas in his truck that he used for a plumbing business. J. Katzakian explained <br /> how his father passed away in 1956 and the tank had not been used since. I <br /> explained the permit requirements associated with removing U.S.T.'s. J. Katzakian <br /> told me he did not realize there were permit requirements associated with removing <br /> this U.S.T. I told J. Katzakian that a permit application needed to be submitted to this <br /> office with the required permit and back fees. I told J. Katzakian I would write down <br /> the requirements on an inspection report and mail it to him. I also told J. Katzakian <br /> how situations in the past where U.S.T.'s were taken out without permits issued from <br /> this office were referred to the District Attorney's Office. I mailed an inspection report <br /> and an application to remove the U.S.T. to J. Katzakian. <br /> On September 8, 1997, 1 received a call from Art Katzakian who asked questions <br /> about completing the application to remove the U.S.T. at this site. I explained to A. <br /> Katzakian how the contractor usually completes the application. When asked, A. <br /> Katzakian explained how he had previously spoken to Doug Wilson about the <br /> removal of U.S.T.'s, and was not aware that a permit was required to remove this <br /> U.S.T. I confirmed the use of the U.S.T. with A. Katzakian who explained the tank <br /> was also used to fuel a tractor used to maintain the property. A. Katzakian explained <br /> if he had known a permit was required to remove the U.S.T., he would have applied <br /> for one. I faxed A. Katzakian a list of contractors who remove U.S.T.'s. <br /> I spoke to Doug Wilson about any recent conversations he could recall about removal <br /> of farm tanks. Wilson told me he had a conversation from somebody who had a <br /> client, who reportedly had a farm tank, and wanted to know of the permit <br /> requirements. Wilson told me he explained the farm tank requirements to the caller, <br /> and how he advised the caller of fire permit requirements regardless of the tank <br /> status. <br /> On September 15, 1997, 1 witnessed the sampling of the U.S.T. excavation by Thorpe <br /> Oil under an approved U.S.T. removal permit issued by this office on September 12, <br /> 1997. <br /> On September 16, 1997, 1 spoke to Bill Lewellen who wanted to tell me the <br /> circumstances as to why he was at this site on September 2, 1997. Lewellen told me <br /> he was at this location at the request of Houston. Lewellen explained that Houston <br /> took out the tank for Dana (potential buyer of the property).
