The following issues were found with the designated operator monthly reports- <br /> --The designated operator failed to document all the alarms from the attached alarm histories on the <br /> October 2012 and January 2012 designated operator monthly inspection reports and failed to check that <br /> they were responded to appropriately. The missing alarms include: 10/25/12 (L14 - 89 STP sump). The <br /> following alarms were noted, but no corrective actions were listed: 1/25/12 (L5 - UDC 9/10), 1/27/12 <br /> (L11 - 87 STP sump), and 12/6/11 (L12 - 89 annular). <br /> -- The alarm history printouts were not attached to the August 2012 or December 2011 monthly reports. <br /> --The facility maintains a log of all monitoring panel alarms. Fifteen alarms were found listed on the daily <br /> alarm logs that were not listed on the monthly alarm history printouts or included in the designated <br /> operator monthly inspection reports. <br /> --The May 2012 designated operator monthly report was dated May 2, 2012. The alarm history printout <br /> attached to the report listed the most recent alarms as April 24, 2012 (Cl - 87 fill sump); this alarm <br /> history did not list the date it was printed. However, the facility daily alarm logs indicate C1 fuel alarms <br /> also occurred on April 25, April 27, April 29, and May 1, 2012. None of these alarms is listed in the <br /> printed alarm history included with the May monthly designated operator report. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous <br /> month, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the monthly report.
