The information on this <br /> monitoring program are conditions of the operating permit. The permit holder must notify <br /> San Joaquin County Environmental Health Division(209)468-3420 within 30 days of any changes to <br /> the monitoring procedures,unless required to obtain approval before making the change. <br /> Required by Sections 2632(d) and 2641(h)CCR. <br /> Facility Name: Pacific Bell <br /> Facility Address: 4051 Newton Road, Stockton <br /> 1. If an unauthorized release occurs, how will the hazardous substance be <br /> cleaned up? Note: if released hazardous substances reach the environment, <br /> increase the fire or explosion hazard, are not cleaned up from the secondary <br /> containment within 8 hours, or deteriorate the secondary containment, then <br /> San Joaquin County Environmental Health Dept. on 209-468-3420 must be <br /> notified within 24 hours. If an unauthorized release occurs contact the <br /> authorized person(s) listed below in #5. They will make arrangements with a <br /> Pacific Bell approved contractor for clean-up. <br /> 2. Describe the proposed methods and equipment to be used for removing and <br /> properly disposing of any hazardous substances. <br /> Immediate Action: <br /> a. Notify a responsible manager. Determine nature, location, <br /> magnitude and severity of incident. <br /> b. Move people away from the area as appropriate. <br /> c. Contain by arranging physical barriers (i.e., dikes,berms,etc.) <br /> Use of sorbents (i.e., neutralizers, oil-absorbent materials, rags, etc.) <br /> 3. Describe the location and availability of the required cleanup equipment in <br /> item 2 above. Spill Control Equipment is located in the Engine Room. <br /> 4. Describe the maintenance schedule for the cleanup equipment. Cleanup <br /> equipment is inspected monthly by the Power Supervisor. <br /> 5. List the name(s) and title(s) of the person(s) responsible for authorizing any <br /> work necessary under the response plan. <br /> Site Manager, Pamela Graham on 209-943-4050 <br /> Emergency Control Center on 510-823-7777 (24 hours a day). <br /> Environmental Management Hotline on 510-823-9833 (8am - 5pm). <br />
