The most recent financial responsibility documents submitted to the EHD are dated 12-7-10. <br /> Current financial responsibility documents are required to be submitted annually. During the inspection, <br /> a current copy of the financial responsibility documents were submitted. Continue to ensure that current <br /> copies of the financial responsibility documents are submitted to the EHD annually. Corrected on site. <br /> 109. Annual monitoring system certification, leak detector testing, and spill container testing was <br /> performed on 12-20-10 and a test report was submitted to the EHD on 2-17-11, almost two months late. <br /> A copy of the test report must be submitted within 30 days of the tests. Continue to ensure that test <br /> reports are submitted within 30 days. <br /> 113. A new designated operator was hired on 10-19-11 and notification was not provided to the EHD <br /> within 30 days of the change. Any changes shall be reported to the EHD within 30 days. During the <br /> inspection a copy of the owner D.O. Statement was submitted. Corrected on site. <br /> 202. Approved copies of the response plan was not found on site. Copies of this pian shall be retained <br /> on site at all times. During the inspection an approved copy of the response plan was provided to the <br /> facility. Continue to ensure that a copy is retained on site. Corrected on site. <br /> 204. The designated operator failed to check and note the correct test dates for the spill container test <br /> on the 8-29-11 and 9-29-11 designated operator monthly inspection reports. The date was marked as <br /> 12-11-10 but should have been marked as 12-20-10. During the monthly inspection, the designated <br /> operator shall check that all required testing and maintenance for the UST system have been completed <br /> and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> 303.
