• • <br /> ' BANKS & CO <br /> E. Any tool with a wooden handle such as a shovel or hammer with a broken or <br /> splintered handle shall not be used. <br /> HOUSEKEEPING <br /> A. Keep and maintain as clean a work area os is reasonably possible. <br /> ' B. Keep small tools, debris, dirt and asphalt chunks away from the edge of <br /> trenches, a minimum of 2 feet, to avoid someone from kicking, pushing or <br /> dropping items on an unsuspecting workman below. <br /> C. Keep the work area behind the main operation as neat and orderly as <br /> possible not only for employees safety, but public safety. <br /> 0. Barricade possible hazards and obstruction when they cannot be alleviated <br /> immediately and are in the path of fellow workmen or the public. <br /> ' TRENCHES AND EXCAVATIONS <br /> A. No one shall enter a trench or excavation that is over 4' in depth, unless <br /> the trench or excavation has been properly sloped or shored. <br /> ' B. Any trench which shows signs of cracking, shifting or similar ir- <br /> regularities, regardless of the depth, shall not be entered. <br /> C. No employee shall ever place himself or another within an excavation which <br /> is doubted to be safe. <br /> ' D. Any trench over 4' in depth must be shored by at least the minimum <br /> requirements or sloped back to at least a 3/4 : 1 angle, or as conditions <br /> require. <br /> E. An authorized qualified supervisor shall direct the shoring operation and <br /> modify as needed to insure maximum worker safety. <br /> F. At the beginning of each shift, the trench and shores shall be inspected <br /> for any unsafe conditions prior to anyone entering the excavation. <br /> ' G. A ladder will be used in all excavations and shall be within 25 feet <br /> (horizontal distance) of any workmen in the trench. The ladder shall be <br /> firmly positioned from the bottom on one side of the trench to the top of <br /> ' the opposite side and extend at least 2' above the ground level surface. <br /> At no time shall the ladder be placed upon the shores or upon any pipe <br /> being installed within the trench. <br /> ' H. Existing utilities shall be located and marked. Utility owners shall be <br /> notified 48 hours in advance prior to trenching within the area of their <br /> lines. <br /> I. Always be alert to the presence of high-voltage lines and never operate <br /> any piece of equipment (ie loader bucket, backhoe boom) within 10 feet of <br /> such lines. <br /> 15 SITE SAFETY PLAN <br />
