BANKS & CO <br /> 3. Keep the work area behind the main operation as neat and orderly as <br /> ' possible not only for employees safety, but public safety. <br /> 4. Barricade possible hazards and obstruction when they cannot be <br /> alleviated immediately and are in the path of fellow workmen or the <br /> public. <br /> 11 . 13 TRENCHES AND EXCAVATIONS <br /> ' 1 . No one shall enter a trench or excavation that is over 4' in depth, <br /> unless the trench or excavation has been properly sloped or shored. <br /> 2. Any trench which shows signs of cracking, shifting or similar ir- <br /> regularities, regardless of the depth, shall not be entered. <br /> 3. No employee shall ever place himself or another within an excavation <br /> which is doubted to be safe. <br /> 4. Any trench over 4' in depth must be shored by at least the minimum <br /> requirements or sloped back to at least a 3/4 : 1 angle, or as <br /> conditions require. <br /> 5. An authorized qualified supervisor shall direct the shoring <br /> operation and modify as needed to insure maximum worker safety. <br /> 6. At the beginning of each shift, the trench and shores shall be <br /> inspected for any unsafe conditions prior to anyone entering the <br /> excavation. <br /> ' 7. A ladder will be used in all excavations and shall be within 25 feet <br /> (horizontal distance) of any workmen in the trench. The ladder <br /> shall be firmly positioned from the bottom on one side of the trench <br /> ' to the top of the opposite side and extend at least 2' above the <br /> ground level surface. At no time shall the ladder be placed upon <br /> the shores or upon any pipe being installed within the trench. <br /> 8. Existing utilities shall be located and marked. Utility owners <br /> shall be notified 48 hours in advance prior to trenching within the <br /> area of their lines. <br /> 9. Always be alert to the presence of high-voltage lines and never <br /> operate any piece of equipment (i .e. loader bucket, backhoe boom) <br /> within 10 feet of such lines. <br /> 10. No unnecessary machinery or heavy loads will be permitted along the <br /> edges of a trench. If it becomes necessary to dump or lower <br /> material into the trench, the workmen below shall vacate to a safe <br /> area until such procedure has been complete. <br /> 11 . If your task is complete in the trench, don't loiter! Vacate to the <br /> top side. <br /> 15 SITE SAFETY PLAN <br />
