He is not listed as a <br />designated operator for the facility and notification was not provided to the EHD within 30 days of the change. Any <br />changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting <br />System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for <br />this facility. <br />This is a minor violation. <br />207 <br />CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br />The designated operator failed to check and note the correct test dates for the Monitoring System Certification, Line <br />Leak Detectors, Spill containers and Secondary Containment tests on the February 2015 through December 2015 <br />designated operator monthly inspection report. The incorrect/ missing dates: <br />- December 2015 report did not have the secondary containment date listed <br />- March 2015 through October 2015 reports did not have any dates listed <br />- February 2015 had incorrect secondary containment test date of 11-18-14. Correct date is 11-19-14. <br />- November 2015 and June 2015 reports not on site <br />During the monthly inspection, the designated operator shall check that all required testing and maintenance for the <br />UST system have been completed and document the dates they were done. Ensure that designated operators <br />performing monthly inspections at this facility are including all of the required information on the reports. This was <br />corrected on site. <br />This is a repeat violation, Class Il. <br />208 <br />CCR 2715 Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />The November 2015 and June 2015 designated operator monthly inspection reports were not found on site. <br />Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate <br />and ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br />maintained on site. Submit copies to the EHD. <br />This is a minor violation. <br />309 <br />CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br />The Bravo box float and chain in the 5-6 UDC sump failed to stop the flow of product at the dispenser when tested. <br />All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at <br />the dispenser when it detects a leak. The service technician adjusted the chain(s) and verified functionality.
