The following is an itemized list of underground storage tank violations that have not <br /> been addressed for Fast Lane Central Valley as of October 16, 2017. <br /> Open violations from June 17,2016 inspection <br /> Violation#201 -Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. 1-11 sensor on the auto side <br /> went into alarm on 8/22/2015, 9/9/2015 and 9/102015, as noted by the designated operator(DO); however,the <br /> alarms were not addressed by the DO and maintenance records associated with these alarms were not found on <br /> site. L22 sensor, on the truck side,went into alarm on 1/27/2016 and went into alarm three times on 5/20/2016 but <br /> the alarms were not documented by the DO nor were maintenance records associated with these alarms found on <br /> site. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and <br /> time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any <br /> visual observations; (4)results of sample analysis perfonned a lab or in the field; (5)logs of all readings of gauges <br /> or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and <br /> reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br /> site and submit copies to the EHD. <br /> Violation 9203-Failed to maintain records of monitoring,repair,lining,and upgrades on site. <br /> Monitoring system certification testing reports were not found on site during the inspection. Also records of repairs <br /> associated with the L11 sensor alarms on 8/22/2015, 9/9/2015, 9/10/2015 and L22 sensor alarms on 1/27/2016 and <br /> 5/20/2016 for the UST system were not found on site. These records shall be kept on site for the life of the tank. <br /> Immediately locate and maintain these records on site and submit copies to the EHD. <br /> Violation#204-Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans was not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> Violation#209-Designated operator did not inspect all required items and/or the inspection reports not <br /> completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the January 29,2016 <br /> and May 28,2016 designated operator monthly inspection reports and failed to check that they were responded to <br /> appropriately. The missing alarms include: 122, brine sensor in dispenser 17A/1 8B on the truck side, on January <br /> 27, 2016 and three times on May 20,2016. During the monthly inspection,the designated operator shall review the <br /> alarm history for the previous month, check that each alarm was documented and responded to appropriately, and <br /> attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> Violation#305-VPH monitoring of the interstitial spaces of the UST system is not maintained. <br /> Communication in the 87-,91-, and three diesel-submersible turbine pump(STP)sumps and fill sumps,and <br /> communication in the under-dispenser-containments(UDC)of the truck side of the facility as well as vacuum sensor <br /> monitoring of the secondary containment piping of the truck side of the facility of the vacuum/pressure/hydrostatic <br /> (VPH)system were not tested during the inspection on June 16 and 17, 2016. The interstitial space of the UST <br /> shall be maintained, as designed, under constant vacuum such that a breach in the primary or secondary <br /> containment is detected before the hazardous substance is released into the environment. Immediately contact a <br /> properly licensed,trained, and certified contractor to test communication of the VPH components listed above under <br /> inspection by the Environmental Health Department(EHD). <br /> Documentation of the S6 diesel STP sensor on the auto side of the facility indicates that the S6 sensor has been <br /> going into alar intermittently. On 6/16/2016, S6 sensor was tested, passed, and closed alarm; however, S6 went <br /> into alar on 6/17/2016. Immediately contact a properly licensed, trained, and certified contractor to repair or <br /> replace the sensor under permit and inspection by the EHD. <br /> Page 6 Of 9 <br />
