No remote afterloading device equipment operation authorized by this license shall be initiated unless the
licensee has in his possession detailed written instructions specific for the make and model of the remote
afterloading device equipment and for the type of operation involved.
38. The licensee shall not transfer any radioactive source or device containing a radioactive source, except in
accordance with the transferee's radioactive material license as to kind and amount of radioactive material,
manufacturer and model number of the radioactive source, and manufacturer and model number of the remote
afterloading device.
39. Each remote afterloading device shall be inspected and serviced in accordance with the manufacturer's
recommendations.
40. Installation, relocation, maintenance, and repair of devices containing radioactive material, and leak testing,
replacement and disposal of sealed sources containing radioactive material used in devices shall be performed
only by persons specifically authorized to do so.
41. Upon initial source loading and/or subsequent to each source exchange, radiation surveys shall be performed
prior to initial human use as follows:
(a) A radiation survey shall be made of:
(1) The unit source housing, with the source(s) in the shielded position. The maximum radiation
levels at 20 centimeters from the surface of the source housing not exceed 3 millimentgens per
hour.
(2) All areas adjacent to the treatment room with the source in the 'irradiate" position. The survey
shall be performed with a phantom in the primary beam of radiation and shall clearly establish:
a. that radiation levels in restricted areas are not likely to cause personnel exposures in
excess of the limits specified in the Code of Federal Regulations, title 10, part 20,section
20.1201.
