The survey <br /> shall be performed with a phantom in the primary beam of radiation and shall clearly establish: <br /> a. that radiation levels in restricted areas are not likely to cause personnel exposures in <br /> excess of the limits specified in the Code of Federal Regulations,title 10,part 20,section <br /> 20.1201. <br /> b. That radiation levels in unrestricted areas do not exceed the limits specified in the Code <br /> of Federal Regulations, title 10, part 20, section 20.1301. <br /> (b) Records of survey results shall be maintained for inspection. <br /> 42. Irrunediately after retracting the source from the patient into its shielded position in the remote afterloading <br /> device, a radiation survey shall be made of the patient and the remote afterloading device with a portable <br /> radiation detection survey instrument to confirm that the source has been removed from the patient. Records <br /> of each survey shall be maintained <br /> 43. Treatment time calculations and data entry for remote afterloaders shall be individually verified by the <br /> responsible physician named on the license immediately prior to treatment. <br /> 44. When engaging the services of a company for the use of a transportable Fligh Dose Rate Afterloader,electrical <br /> interlock(s) on entrance door(s)to the remote afterloading device room(s) shall be tested for proper operation <br /> prior to the first treatment of the day. Records of test results shall be maintained for inspection. <br /> 45. When engaging the services of a company for the use of a transportable High Dose Rate Afterloader, the <br /> licensee shall retain all responsibilities for the use of radioactive material authorized by this license. <br /> 46. This license does not authorize commercial distribution of radioactive material. <br /> 47. Production or processing of radiopharmaceuticals for the purpose of distribution to other licensees is not <br /> authorized by this license. <br /> 48. Radioactive materials shall be used by occupational workers in such a manner that the dose limits specified in <br /> Title 10,Code of Federal Regulations,Part 20,Subpart C(Sections 20.1201 through 20.1208)are not exceeded. <br /> 49. The licensee shall monitor occupational exposures to radiation and shall supply and require the use of individual <br /> monitoring devices by personnel as required by Title 10,Code of Federal Regulations,Part 20,Section 20.1502 <br /> (a). <br /> 50. The licensee shall monitor occupational intakes of radioactive material by,and assess the committed effective <br /> dose equivalent to, individuals who may have exceeded or are likely to exceed,the limits specified in Title 10, <br /> Code of Federal Regulations(CFR), Part 20, Section 20.1502(b). Suitable and timely measurements used for <br /> determination of such internal exposures shall be performed as specified by 10CFR 20.1204. <br /> 51. This license does not authorize distribution to persons licensed pursuant to Section 30195 (a) and (b) of the <br /> California Radiation Control Regulations or equivalent provisions of the NRC or Agreement States. <br />
