EMERGENCY REPORT INFORMATION <br />The following minimum information must be included in written reports to the <br />Agency following notification of an emergency situation (fire, reportable spill, <br />etc.): <br />1. Name, address and phone number of the owner/operator; <br />2. Name, address and phone number of the facility; <br />3. Date, time and type of incident; <br />4. Name & quantity of material(s) involved; <br />5. The extent of injuries, if any; <br />6. Description of any property or equipment damage; <br />7. An assessment of actual or potential hazards to human health or <br />the environment, where this is applicable; <br />8. Estimated quantity and disposition of recovered material that <br />resulted from the incident. <br />
