—Advanced Interconnect Technologies <br />Emergency Response Plan <br />Page 33 <br />EMERGENCY NOTIFICATION PROCEDURE <br />The following minimum information must be provided whenever Agency notification of a <br />emergency situation (fire, reportable spill, etc.) is required. <br />1. Name and phone number of the individual making the telephone report. <br />2. Name and address of the facility where the incident occurred. <br />3. The date and time of the incident; duration of release. <br />4. Type of accident/incident (spill, explosion, fire, etc.) <br />5. Name of any hazardous material(s) involved, approximate quantity <br />released, and where material released (i.e. to air, land, water) <br />6. The number and extent of any injuries. <br />7. If known, the potential direct or indirect hazards presented by the <br />hazardous material to the environment and humans both inside and <br />outside the facility. Include - <br />• Whether individuals have been contaminated; <br />• Whether facility is being evacuated; <br />• Whether surrounding area should be evacuated; <br />• Any advice about medical attention necessary for exposed <br />individuals; <br />• Any known (or anticipated) acute or chronic health effects from the <br />hazardous material. <br />
