Reyes SUBJECT: Appendix A to Certification of Compliance Notice APPENDIX A The following list includes summaries and attachments for conformance to the DTSC Summary of Violations issued to AIT on 11/28/00. Each summary and attachment are listed in the same numerical order as listed in the "Summary of Violations". Section 1 "Summary of Violations" conformance actions. 1. a.) AIT contacted Doctor's Hospital on Manteca on 12/20/00 and 12/22/00. Please see first attachment. b.) List has been modified to Appendix C of the HMMP to correlate Emergency Personnel with their title(s). Please see Appendix C of the attached HMMP. c.) Phone list contained in Appendix F of the HMMP was modified to include State OES phone number. Please see Appendix F of the HMMP. 2. Physical states were added to all of the hazardous waste satellite and storage area drums. Please see attachment 2 which contains a picture of one of the twelve correctly label drums and attachment 3 which is a copy of the new label that will be used which includes a physical state check box. Attachment 4 contains a copy of the training log covering proper labeling of hazardous waste drums. 3. The bung was secured on the satellite drum immediately. Attachment 5 contains a picture of the secured drum and Attachment 4 contains a copy of a training log covering proper securing of hazardous waste drums. 4. Please see attached copy PBR notification, business activities, and business/owner/operator. Original is attached to the San Joaquin County EHS copy for their records. 'Bottom of Page 3 of 4. Any raw material non -labeled drums were immediately labeled. Please see attachment 4 for a copy of the training log covering proper labeling of raw materials. Section 1 "Summary of Violations" conformance actions. 1. Safety-Kleen was immediately contacted and provided a copy of the signed transporter manifest for Manifest # 20532578. Attachment 6 is a copy of the signed manifest. AIT is currently working with ATC Associates, Inc. on getting the required certification. Please see attachment 7 in regards to the status of the certification.
