Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />July 10, 2018 <br /> Facility Address: <br /> 804 N Cluff Ave, Lodi <br /> Facility Name: <br /> Valley Mountain Auto Service ,Inc. <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sjgov.org/ehd <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 101 CCR 66262.12 Managed a hazardous waste without an identification number. <br />This facility does not have a valid ID number to manage hazardous waste. A hazardous waste generator shall not <br />treat, store, dispose of, transport or offer for transportation, hazardous waste without an ID number. If you or your <br />business generates more than 100 kg of RCRA hazardous waste and/or more than one kg of acutely hazardous <br />waste, then you need to obtain an EPA ID number. All others must obtain a State ID number. Go to the DTSC web <br />site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to apply for an ID number. Immediately obtain <br />an ID number to manage hazardous waste and submit evidence to the EHD. <br />This is a minor violation. <br /> 104 CCR 66262.34(d)(2) No modified contingency plan. <br />The modified contingency plan lacked the location of the spill control equipment. There must be at least one <br />emergency coordinator on site or on call to coordinate emergency response measures, and the following <br />information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br />extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, <br />unless the facility has a direct alarm. Immediately include the location of the spill control equipment on the posted <br />modified contingency plan. A form is provided that can be used for this purpose. <br />This is a minor violation. <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br />Copies of uniform manifests for the disposal of hazardous waste for the past three years were not found on site. <br />Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br />used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br />Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit <br />copies to the EHD. <br />This is a Class II violation. <br /> 111 CCR 66262.42(a)(c)(d) Failed to comply with uniform hazardous waste manifest exception requirements. <br />Copies of uniform manifests for the disposal of hazardous waste for the past three years were not found on site. <br />Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the <br />destination facility is received. The signed off copy shall be retained and made readily available for review for three <br />years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator <br />of the facility to which the generator’s waste was submitted within 60 days of the date the waste was accepted by <br />the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the <br />generator has not received confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O.
