III. OWNER/OPERATOR: <br /> E.R. Carpenter is an international corporation operating in the U.S.A. and <br /> Canada. The facility is one of eight plants which the company operates. <br /> Steve Setzer is the Facility Manager and Steve Fabela is the Division <br /> Manager. The home office is located at 5016 Monument Avenue, P.O. Box <br /> 27205, Richmond Va, 23261. <br /> IV. BACKGROUND: <br /> The facility was first opened in 1986. The first hazardous waste inspection <br /> was conducted by Jamie Favila, PHS-EHD, in August, 1989. Local hazardous <br /> waste generator fees are current at the site and the facility is categorized as <br /> a hazardous waste generator producing greater than 5 tons and less than 25 <br /> tons of hazardous waste per year. In 1992 the facility was inspected by <br /> Eleanor Ratliff, PHS-EHD, on June 10. The following types of violations were <br /> noted on the October 19, 1992 Report of Violations: <br /> * Personnel training records were not available. <br /> * The contingency plan was incomplete. <br /> * Land disposal restriction forms were missing. <br /> * All manifests were not available for inspection. <br /> * Three violations for open hazardous waste containers were noted. <br /> * Four labeling violations were noted. <br /> * Decontamination equipment was not available in one hazardous waste <br /> storage area. <br /> A Certification Statement was submitted by E.R. Carpenter to demonstrate <br /> compliance on November 9, 1992. <br /> On March 16, 1993 a diesel spill was reported. Approximately 30 gallons of <br /> fuel was spilled and a small amount entered the storm drain. E.R. Carpenter <br /> activated their contingency plan, and the spill was promptly cleaned up. Eric <br /> Trevena of PHS-EHD made all necessary notifications and oversaw the <br /> cleanup efforts. <br /> E.R. Carpenter uses City of Lathrop public drinking water inside the plant, <br /> however an irrigation well for watering the landscape is used outside. A <br /> septic system is used instead of public sewer even though public utilities <br /> exist in the area. A storm water retention pond is located east of the <br /> warehouse and is permitted by the California Water Quality Control Board. <br /> The facility I.D. number is 5B39S006816. <br /> 2 <br />
