Violations <br /> #5, #9, #12: Effective immediately, all hazardous waste containers shall be closed <br /> except when adding or emptying waste material. <br /> Violation #6: Within 10 days of receipt of this report, all areas where hazardous <br /> waste is stored shall have emergency employee decontamination <br /> equipment including, but not limited to, eye wash equipment. <br /> Violations #7 <br /> #8, #10,#11: Effective immediately, all hazardous waste containers shall be <br /> labeled with the following information: <br /> a) The start date of accumulation. <br /> b) The words "Hazardous Waste". <br /> c) Composition and physical state of the waste. <br /> d) Statement or statements which will call attention to the <br /> particular hazardous properties of the waste. <br /> e) Name, address, and phone number of the waste generator. <br /> When all violations have been corrected, please send written certification to this office at <br /> the letterhead address.
