Description of actions facility personnel will take in response to fires, explosions, or any sudden or <br />non-sudden release of hazardous waste to air, soil or surface water at the facility; <br /> 2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br />State and local emergency response teams to coordinate emergency services; <br /> 3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency <br />coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and <br />all others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br />If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br />plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br />are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to <br />fulfill the requirements of this chapter. Submit a copy of the contingency plan to the EHD. <br />This is a minor violation. <br /> 114 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br />Manifest 014057553JJK for 1/5/17 was missing a signature and date from the receiving facility. The generator and <br />waste sections of the uniform manifest shall be completed and signed and dated by the generator. The transporter <br />shall also sign and date the manifest. Ensure that all manifests are properly completed for any transport of a <br />hazardous waste for off-site transfer, treatment, storage or disposal. <br />This is a Class II violation. <br /> 119 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br />Copies of hazardous waste disposal records for 2016 were not found on site. Some recycling receipts for used oil <br />appeared to be missing including used oil receipts for 9/2/16 and 10/18/16. Hazardous waste generators shall retain <br />copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting <br />procedure on site for three years and have them readily available for review.
