The eye wash solution in the hazardous waste and chemical storage room has an expiration date of <br /> April 2004. All facility communications and alarm systems, fire protection equipment, spill control <br /> equipment, and decontamination equipment, shall be tested and maintained as necessary to assure its <br /> proper operation in time of emergency. Immediately conduct all necessary maintenance on this <br /> equipment. <br /> 67. No waste analysis was available for the sand blaster waste. This waste is being hauled as non- <br /> hazardous, designated waste. Records indicate that it has previously been hauled as hazardous. A <br /> generator shall keep a record of any test results, waste analyses, or other determinations made in <br /> accordance with hazardous waste regulations for at least three years from the date that the waste was <br /> last sent to on-site or off-site treatment, storage, or disposal. Immediately locate a copy of the waste <br /> analysis conducted for the sand blast waste and submit a copy to the EHD by July 23, 2011. <br /> 83. Used fluorescent bulbs were observed without the words "Universal Waste." A generator shall label <br /> or mark universal waste with the words 'Universal Waste' and the type of waste (alkaline batteries, <br /> mercury containing equipment, fluorescent bulbs, electronic devices, CRTs, aerosol cans). Immediately <br /> label these wastes and ensure that all universal wastes are labeled with the required information. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by July 23, 2011 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> "Facility generated 0.54 tons of hazardous waste in 2010. The facility will be moved to a lower permit <br /> category (<5 tons/year). This may result in decreased permit fees. <br /> Note: (corrected on site) <br /> The emergency coordinator contact information on the modified contingency plan is outdated. There <br /> must be at least one emergency coordinator on site or on call to coordinate emergency response <br /> measures, and the following information must be posted by a phone: the name and phone number of the <br /> emergency coordinator, location of fire extinguishers, spill control equipment, and if present, fire alarm; <br /> and the phone number of the fire department, unless the facility has a direct alarm.
