Box 2009• (1601 East Hazelton Avenue) •Stockton,California 95201 <br /> (209)468-3400 <br /> April 24, 1991 <br /> Mr. Dalton Beitz <br /> Lodi Metal Tech Inc. <br /> Lodi, CA. 95240 <br /> RE: REPORT OF VIOLATIONS AND SCHEDULE OF CORRECTIONS <br /> On January 19, 1990, Public Health Services-Environmental Health Division (PHS-EHD) conducted an <br /> inspection of Lodi Metal Tech Inc., located at 213 S. Kelly Street in Lodi. <br /> As a result of that inspection,violations of the hazardous waste status and/or regulations were identified. <br /> Specific violations and required corrective action are listed below. Failure to correct the identified <br /> violations within the schedule period will result in PHS-EHD citing you for continuing/additional <br /> violations. <br /> I. REPORT OF VIOLATIONS: <br /> A. Section 66508 (a) (2), and (3),Title 23, of the California Code of Regulations. <br /> 1) At the time of inspection, there were approximately sixty - 55 gallon drums of <br /> paint sludge which did not have labels or accumulation start dates. <br /> 2) At the time of inspection, there was one - 55 gallon drum of waste oil without a <br /> label or accumulation start date. <br /> B. Section 67105, California Code of Regulations. <br /> 1) At the time of inspection LMT had not implemented a personnel training <br /> program. <br /> C. Section 67140 (a), California Code of Regulations. <br /> 1) At the time of inspection LMT had an inadequate contingency plan. Plan does <br /> not include personal protection for cleanup of 1,1,1, Trichloromethane. <br /> 1 <br /> A Division of San Joaquin County Health Care Services 10 <br />
