SCHEDULE OF CORRECTIONS: <br /> Violation #1: Effective immediately all drums containing hazardous waste shall be labeled with <br /> the following information. <br /> a) The accumulation start date. <br /> b) The words "Hazardous Waste" <br /> C) The composition and physical state of the waste. <br /> d) Statement or statements which will call attention to the particular <br /> hazardous properties of the waste. <br /> e) Your name and address. <br /> Violation #2: Within 30 days of receipt of this report a training program specifically addressing <br /> hazardous waste handling shall be implemented. <br /> Violation #3: Within 30 days of receipt of this report the contingency plan shall be updated. <br /> When all violations have been corrected, please send written certification to this office at the letterhead <br /> address. This certification shall contain the sample certification statement indicated on Attachment 1. <br /> Public Health Services will conduct a reinspection of your property to verify compliance.
