San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax: (209)468-3433 Web:www.sjgov.org/ehd <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> SPACERAK 214 S KELLY ST, LODI March 19. 2015 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS 11,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 105 CCR 66262.11, 66262.40(c) Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 <br /> years. <br /> A non empty aerosol can of solvent was left in the trash drum of bay one across from pole 11 prior to making a Title 22 <br /> hazardous waste determination for it. Any person who generates a waste shall determine if the waste is a hazardous <br /> waste. During the inspection this aerosol can of solvent was placed on the shelf for continued use. Immediately <br /> continue to make a hazardous waste determination for any waste generated by this business, and manage it <br /> according the Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining <br /> how this type of waste will be managed. <br /> This is a repeat violation, Class II <br /> 15 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> Manifest number 008642929JJK was found without a signed off copy frorn the destination facility. Hazardous waste <br /> generators shall retain copies of all manifests signed off by the destination facility on site for three years and have <br /> them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature <br /> of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the <br /> waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator <br /> of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off <br /> copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator <br /> shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the <br /> generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall <br /> be submitted to. <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. By 4-19-15, <br /> submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. <br /> This is a repeat violation, Class Il. <br /> 01 CCR 66265.31 Facility not maintained to minimize the release of a hazardous waste. <br /> Shop at lot 213 has a shelf on the west side of the shop towards the back wall of the shop. This shelf has parts that <br /> that have leaked lubrication fluid onto the shop floor below the shelf. It appears that the spilled lubrication fluid has not <br /> been cleaned up and that oily debris has been left to accumulate there. Just east of this shelf is a 75 ton P8 press <br /> that has an accumulation of oily debris below it on the shop floor. Facilities shall be maintained and operated to <br /> minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could <br /> threaten human health or the environment. Immediately clean up the spilled lubrication fluids and oily debris, and <br /> manage according to Title 22 hazardous waste regulations.
