TELEPHONE / VERBAL
Fmcon
CONVERSATION RECORD
DATE May 4, 1988 PROJECT NO. 174-18.01
PROJECT NAME Storm Water Collection Pond Assessment
CLIENT Stockton Scavenger PHONE (209 468-3440
CLL FROM Mark Thomas -CALL To David Carls - Environmental Health
OR
PARTIES PRESENT AT CONFERENCE
SUMMARY OF CONVERSATION
I called to clarify the purpose of the Authorization to Release Analytical
Result, form flavin Carl , cant mp Hp explained that thi, is to inuira
he gets analytical results once the storm water pond assessment is completed.
Mr. Carls also said I should call Ms. Antona Vorster at the RWOCB
(916) 361-5600. He advised me that the RWQCB will eventually become involved
with the pond assessment and that it is best to involve them from the
beginning
He also said that I need to complete a General Fnvirnnmpntal Hpalth Application
Form and pay a $35 fee when the pond assessment plan is submitted. The purpose
is to establish an account against which the environmental health department
will bill its time for reviewing the plan and subsequent reports This is
becoming a common way for local environmental health departments to fund
their agency.
PREPARED BY Mark Thomas
DISTRIBUTION: Project File, Jim Walsh, Jim Wyse
