Hazardous <br /> waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. <br /> The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of <br /> the manifest with the handwritten signature of the owner or operator of the facility to which the generators waste was submitted <br /> within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing <br /> manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit the required information to DTSC. Submit to the EHID a • <br /> copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. <br /> See Appendix F. <br /> 703. An emergency coordinator and modified contingency plan information is lacking. There must be at least one emergency <br /> coordinator on site or on call (available to respond to an emergency by reaching the facility within a short period of time)to <br /> coordinate emergency response measures, and the following information must be posted by a phone: the name and phone number <br /> of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number <br /> of the fire department, unless the facility has a direct alarm. This emergency coordinator shall be thoroughly familiar with: <br /> - all aspects of the facility's contingency plan (and have the authority to commit the resources needed to carry out the contingency <br /> plan) <br /> - all operations and activities at the facility <br /> - the location and characteristics of waste handled <br /> - the location of all records within the facility <br /> - the facility layout <br /> Immediately appoint an emergency coordinator and post the required information by a phone.
