Hwy 26 Stockton, CA 95215 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />120. An <br />inspection was last done on 6-26-15. An inspection report was issued identifying information to <br />be submitted to bring this site into compliance. This information was required to be submitted by <br />7-26-15. <br />This information has not been received resulting in a non-compliant status for this facility. An <br />operator <br />that receives an inspection report shall have 30 days of receiving an inspection report. The <br />6-26-15 <br />violations include the following: <br />101. <br />No permit to operate. <br />103. <br />Current financial responsibility documents not submitted. <br />104. <br />UST Operating Permit Application for Facility and Tank information not submitted or current. <br />105. <br />Current BOE number not submitted. <br />106. <br />Monitoring and response plans not current or approved by the EHD. <br />107. <br />Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br />109. <br />Failed to test secondary containment. <br />111. <br />Failed to test monitoring and leak detection equipment annually. <br />112. <br />Failed to test spill containers annually. <br />115. <br />Failed to inform the EHD of a change of designated operator within 30 days. <br />125. <br />Failed to submit a written response within 30 days of receiving an inspection report. <br />126. <br />Failed to report program data electronically. <br />201. <br />Failed to maintain alarm logs and/or records of follow up actions. <br />203. <br />Failed to maintain records of monitoring, repair, lining, and upgrades on site. <br />204. <br />Current monitoring plan approved by the EHD not found on site. <br />205. <br />Operating permit not on site. <br />207. <br />Designated operator did not inspect all required items and/or inspection reports not completed. <br />208. <br />Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />209. <br />Designated operator employee training not performed o log not kept. <br />301. <br />UST system is not operated to prevent unauthorized release, including spills and overfills. <br />302. <br />UST system incapable of detecting leak from primary into secondary containment (pre -July <br />2003). <br />306. <br />Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br />307. <br />Monitoring system of the interstitial space of a double walled system not maintained. <br />308. <br />Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br />309. <br />Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the <br />UDC. <br />310. <br />Sensor in a turbine sump failed to activate an audible and visual alarm or stop the flow product. <br />323. <br />Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />326. <br />Overfill prevention system not present was overridden, or failed to meet overfill requirements. <br />ALL <br />EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />-7 <br />EHD Inspector: <br />Aris Veloso (209) 468-3453 <br />ReceivGdABy: <br />VrIWIUII� <br />itle: <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
