An inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 7-26-15. This information has not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days of receiving an inspection report. The 6-26-15 violations include the following: 101. No permit to operate. 103. Current financial responsibility documents not submitted. 104. UST Operating Permit Application for Facility and Tank information not submitted or current. 105. Current BOE number not submitted. 106. Monitoring and response plans not current or approved by the EHD. 107. Plot plan/site map not submitted or failed to completely show where monitoring is performed. 109. Failed to test secondary containment. 111. Failed to test monitoring and leak detection equipment annually. 112. Failed to test spill containers annually. 115. Failed to inform the EHD of a change of designated operator within 30 days. 125. Failed to submit a written response within 30 days of receiving an inspection report. 126. Failed to report program data electronically. 201. Failed to maintain alarm logs and/or records of follow up actions. 203. Failed to maintain records of monitoring, repair, lining, and upgrades on site. 204. Current monitoring plan approved by the EHD not found on site. 205. Operating permit not on site. 207. Designated operator did not inspect all required items and/or inspection reports not completed. 208. Designated operator monthly inspection reports not maintained on site for at least 12 months. 209. Designated operator employee training not performed o log not kept. 301. UST system is not operated to prevent unauthorized release, including spills and overfills. 302. UST system incapable of detecting leak from primary into secondary containment (pre -July 2003). 306. Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. 307. Monitoring system of the interstitial space of a double walled system not maintained. 308. Failed to maintain a continuous monitoring system that activates an audible and visual alarm. 309. Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. 310. Sensor in a turbine sump failed to activate an audible and visual alarm or stop the flow product. 323. Leak detector failed to detect a 3.0 gph leak at 10 psi. 326. Overfill prevention system not present was overridden, or failed to meet overfill requirements. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
