Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Copies <br /> of these inspection reports were emailed to the facility by the designated operator at the time of inspection. This was <br /> corrected on site. <br /> This is a repeat violation, Class 11. <br /> 307 CCR 2632(c) Monitoring system of the interstitial space of a double walled system not maintained. <br /> The diesel annular sensor failed to activate an audible and visual alarm when tested and, therefore, the tank was not <br /> monitored. The interstitial space shall be monitored with a monitoring system connected to an audible and visual <br /> alarm system. The technician smacked the sensor several times and it began working. This was corrected on site. <br /> This is a Class 11 violation. <br /> 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The Bravo box float and chain in the Dispenser 718 (W) UDC sump failed to stop the flow of product at the dispenser <br /> when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br /> product at the dispenser when it detects a leak. The service technician adjusted the chain(s)and verified functionality. <br /> This was corrected on site. <br /> This is a repeat violation, Class ll. <br /> 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 87 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting <br /> a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed_ This <br /> was corrected on site. <br /> This is a repeat violation, Class 11. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 23, 2015. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
