A copy of these plans shall be <br /> accessible on site at all times. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> According to the Environmental Health Department(EHD)records,escrow closed for this business on 4/3/2017. <br /> The permitting documentation was not changed within 30 days of escrow. Immediatly send the proper <br /> documentaion to the EHD. <br /> This is a Class II violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator(DO)failed to check and note the correct test dates. <br /> On the 3/29/2017 DO monthly inspection report,7/27/2016 was incorrectly entered as the monitor certification date <br /> and spill container test.The Correct date is 6/27/2016 <br /> On the 3/29/2017,4/6/2017,5/8/2017 and 6/15/2017 DO monthly inspection reports, 10/17 2013 is incorrectly <br /> entered as the secondary containment testing date. The correct date is 10/11/2016. <br /> During the monthly inspection,the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and document the dates they were done. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> This is a Class II violation. <br /> 211 CCR 2715(f)Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Khalilullah Noorulhuda was not documented in the training log. The <br /> designated operator shall Vain facility employees for which he or she is responsible in the proper operation and <br /> maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner Consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
