An approved copy of the response plans were not available for inspection. A copy of these plans shall be accessible on site at all times. This was corrected on site. This is a Class II violation. 206 CCR 2712 Failure to comply with UST operating permit conditions. According to the Environmental Health Department(EHD)records,escrow closed for this business on 4/3/2017. The permitting documentation was not changed within 30 days of escrow. Immediatly send the proper documentaion to the EHD. This is a Class II violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator(DO)failed to check and note the correct test dates. On the 3/29/2017 DO monthly inspection report,7/27/2016 was incorrectly entered as the monitor certification date and spill container test.The Correct date is 6/27/2016 On the 3/29/2017,4/6/2017,5/8/2017 and 6/15/2017 DO monthly inspection reports, 10/17 2013 is incorrectly entered as the secondary containment testing date. The correct date is 10/11/2016. During the monthly inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a Class II violation. 211 CCR 2715(f)Designated operator employee training not performed or log not kept. The designated operator employee training for Khalilullah Noorulhuda was not documented in the training log. The designated operator shall Vain facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner Consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD.
